TO REQUEST A $75 ($150 couple) POLITICAL CONTRIBUTION REFUND (PCR):

Step 1 - PLEASE EMAIL info@jayxiong.org stating you are requesting for a PCR RECEIPT from the Jay Xiong for MN House Committee. Please include the name(s) and mailing address of the person(s), requesting the PCR in your e-mail.

Step 2 - Once you receive the PCR RECEIPT, complete a PCR APPLICATION. Click HERE to print your PCR APPLICATION.

Step 3 - Mail your PCR RECEIPT with the filled PCR APPLICATION to the MN Department of Revenue address provided on the PCR application. You will receive a direct deposit refund from the state within weeks.
